New Web-based Installation Report and Support Request Forms
Installation Report
Control-X Medical is required by law to keep record of its product installations. We therefore kindly request that you complete the simple Installation Report within 15 days forevery installation. This is crucial as not doing so may affect warranty claims.
Support Form
To improve our support processes, we launch the new ‘Support Request Form’ which will channel all support-related issues (service request, shipment issues, parts returns) through one easy-to-use application. Using this convenient tool, you can:
- request service in case a failure happens;
- report a shipment issue - missing or wrong part / item;
- request an RMA to return goods to Control-X Medcial.
